Form Instructions

Please use this form to register attendees for our summit. If there are several attendees from your organization or group wishing to pay together, please make sure all attendees are added before proceeding to payment information:

  1. Add each attendee by selecting add under the appropriate registration type and providing the requested information. Your invitation letter specifies how many attendees we will sponsor from your agency. If you did not receive an invitation letter, add attendee(s) under Standard Registration.
  2. If you received a sponsored attendee code, apply it now.
  3. Add a donation amount (optional).
  4. After all attendees in your group are added, click continue.
  5. On the following page, first click the dropdown to verify that information is correct for all attendees. Then, fill in payee and payment information to pay for all attendees and click process to finalize your registration. You will receive a confirmation email shortly.

If you prefer to pay by check, please follow these instructions:

  1. Follow the above instructions to add all attendees you want to register
  2. On the payment page, select ‘Check’ as the payment type, then finalize registration. You will receive a bill in your confirmation email.
  3. List names of all attendees with check so we can process them accurately
  4. Make checks payable to: “Asian Pacific Institute on Gender-Based Violence” and mail within 7 days to:
